• As the healthcare industry shifts to Value-Based Payment, stakeholders recognize that patient treatment often goes beyond medical care; there are social determinants of health that impact a patient’s wellness. Community Based Organizations (CBOs) have the expertise to address these social determinants of health including food, housing, transportation, childcare, employment and/or education. These agencies and organizations will play an important role in Value-Based Payment arrangements.

• In an effort to prepare CBO partners to position themselves for success relating to New York State Department of Health Value Based Payment Roadmap, Millennium Collaborative Care is partnering with Community Partners of WNY to offer the following unique training program offering VBP technical assistance for CBOs.

Value Based Proposition Program
• This is a collaborative effort between MCC and Community Partners of WNY. This 4-hour workshop will assist organizations to identify and develop their value propositions(s) and recognize and respond to business opportunities. Organizations will receive technical assistance from Value Based Proposition expert, Kristin Woodlock.

Kristin M. Woodlock RN, MPA, CEO, Woodlock & Associates will serve as the Presenter. Kristin has over 30 years of public service in County and State Government. She served in various executive roles in the New York State Office of Mental Health, including Commissioner. Kristin currently consults with CBOs, Behavioral Health and Health Care Providers across the Country on operational strategies for value-based payment.
