Millennium Collaborative Care to Host Nov. 6, 2019 Conference:
‘Value Based Payment Collaboration: Finding Your Community-Based Organization Social Determinants of Health Partner’
Millennium Collaborative Care will host healthcare providers, insurers and community-based organization partners at the first conference in Western New York to focus on the key role Social Determinants of Health (SDOH) play in Value-Based Payment collaboration and contracting.
A highlight of the conference will be a 90-minute “CBO Showcase” when Community-Based Organizations from across Western New York’s eight counties will be stationed at exhibit tables and have a unique opportunity to:
• Showcase organization’s service offerings, capabilities, and achievements in Western New York.
• Communicate organization’s Value Proposition.
• Engage clinical conference attendees around value-based payment arrangements.
“This conference is another major investment in the transformation to Value Based driven healthcare vs. fee-for-service,” said Al Hammonds, CSSBB, Executive Director, Millennium Collaborative Care. “By bringing together healthcare service providers, health insurers and community-based organizations that have specific, quantifiable value propositions; our goal is to facilitate new partnerships that will address patients’ social determinant of health issues such as insufficient housing, food insecurity, unemployment, lack of transportation and other challenges in a clinically integrated manner that truly addresses the whole person, and in the process, leads to compensation for value-based outcomes.”
