Feb. 8 ‘Community Connections’ Briefing on Partnerships to Reduce Avoidable Hospital Utilization Among Pediatric Populations

Millennium’s Sandra McDougal to present on efforts to reduce poor pregnancy outcomes and improve maternal and child health in Western New York

The Greater New York Hospital Association (GNYHA) continues its Community Connections series with an event on hospital/community-based organization (CBO) partnerships developed to decrease avoidable hospital utilization for pediatric populations on Feb. 8, 2017.

The briefing is part of GNYHA’s ongoing efforts to support hospitals as they partner with CBOs to implement population health improvement and avoidable admission reduction initiatives such as the Delivery System Reform Incentive Payment (DSRIP) program.

During the briefing, Maria D’Urso, Administrative Director, Community Medicine, NewYork-Presbyterian/Queens will discuss the hospital’s work with partner organizations St. Mary’s Home Care and the Asthma Coalition to expand home-based self-management asthma treatments for the Flushing, Queens pediatric population.

Sandra McDougal, Project Manager, Millennium Collaborative Care (MCC), a DSRIP performing provider system led by Erie County Medical Center, and LuAnne Brown, Chief Executive Officer, Buffalo Prenatal-Perinatal Network, will discuss activities undertaken by MCC and four CBOs to reduce poor pregnancy outcomes and improve maternal and child health in Western New York.

Presenters will be joined by representatives from their respective CBO partners.

Representatives from the United Hospital Fund and the New York City Department of Health and Mental Hygiene will also share their respective work to facilitate hospital/CBO partnerships to address pediatric wellness. Presenters will discuss visions for supporting CBO services in the context of value-based payment.
