Millennium Collaborative Care: A Catalyst for Healthcare Change in WNY
As part of our ongoing efforts to improve and increase our communications to our highly-valued stakeholders, I’m very pleased to introduce this first edition of our new “Quarterly Update.”
Millennium Collaborative Care (MCC), an affiliate of the Erie County Medical Center, is a Performing Provider System (PPS) under New York State’s Delivery System Reform Incentive Payment (DSRIP) program, a statewide initiative championed by Governor Andrew M. Cuomo to fundamentally restructure the health care delivery system by reinvesting in the Medicaid program, with the primary goal of reducing avoidable hospital use by 25% over 5 years.
Based in Buffalo, NY, the mission of Millennium Collaborative Care is to collaborate and partner with physicians, healthcare providers and community-based organizations on the implementation of 11 DSRIP programs designed by Millennium to positively impact Medicaid patients across Western New York’s eight counties (Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, Wyoming).
Our objectives via this Update are to provide a topline overview of key strategies and program initiatives we are implementing to achieve our DSRIP goals.
We are also very pleased to further introduce several of our organization and program area leaders; and provide them an opportunity to report on recent progress and recognition highlights, while also sharing a snapshot of key activities and actions they have planned for the Quarter ahead.
Overall, it’s an exciting time for Millennium!
Not only is our organization continuing to rapidly grow and develop in virtually every aspect of our operations; but our teams – in close collaboration with our highly valued partners — are driving initiatives that are yielding meaningful, measurable results.
