Collaborating With Partner Agencies Across Eight Counties

Millennium’s Behavioral Health Team has been working with the variety of partner agencies across the eight county region. The Behavioral Health Team oversees three projects including Behavioral Health Integration, Community Crisis Stabilization and Mental and Emotional Behavioral Well-Being.
BH Integration Update:
The Behavioral Health Integration Team has embarked on a pilot project with some of the larger BH organizations including Horizon, Spectrum Human Services, Lake Shore Behavioral Health, and Mid-Erie along with our local Managed Care Organizations and are focused on data sharing.
The group hopes that through this partnership with our partner PPS Community Partners of WNY, our partners will be better prepared to serve high-risk clientele as additional information on healthcare diagnoses and healthcare needs are identified.
This subcommittee work will continue through 2016. In addition, Millennium Transformation Specialists have been scheduling initial meetings with Primary Care partners to assess their PCMH Level 3 2014 status. They have already met with our SNAPCAP Providers and GBUAHN and will expand their meetings throughout this quarter. These assessments will help towards the DSRIP goal to integrate Behavioral Health into Primary Care offices. Training plans are being finalized for both Model 1 (integrating BH into PC Offices) and Model 2 (integrating PC into BH offices) projects.
Community Crisis Stabilization Update:
All eight County’s Department of Mental Health Commissioners have been essential with building a comprehensive gaps and barriers analysis for hotline, mobile outreach, respite/peer services and substance abuse services across the eight county region. In addition, representatives from partner agencies such as Crisis Services, HOME, Buffalo Psychiatric Center, Community Missions, Restoration Society, and Northpointe Council have been participating in bi-monthly workgroups to pinpoint needs and gaps and to prioritize how to fill the gaps identified.
Millennium representatives are also participating in opiate task forces in both Erie and Niagara counties. In the next quarter, Millennium will be working with our partner PPS from the Finger Lakes Region to coordinate a plan to fill the gaps in services.
Mental and Emotional Behavioral Well-Being:
Millennium has been working alongside Community Partners of WNY to ensure the expanded delivery of evidence-based programs and practices throughout our region.
The Erie County Council for the Prevention of Alcohol and Substance Abuse is the lead agency working with Millennium and CPWNY to help plan and implement a comprehensive array of school and community-based educational programs with partner agencies includingNorthpointe Council, The Chautauqua Council on Alcoholism, CaRES, Genesee-Orleans Council, WNY United Against Drug and Alcohol Abuse, and PreventionFocus.
The Mental Health Association of Erie County is taking the lead with mental health promotion activities in the community. Along with their partner agencies such as Compeer and Jewish Family Services, they are implementing “Mental Health First Aid” sessions and other community-based forums. The team is concurrently working with partner agencies to establish a community wide anti-stigma campaign to try and change the pervasive negativity towards those with mental health and/or substance abuse issues
