Millennium Collaborative Care, PPS and Lead PPS entity Erie County Medical Center Corporation (ECMCC) have released a Request for Proposals (RFP) for the Patient Activation Measure (PAM) and community engagement.  The project involves three primary concepts:

  • Patient Activation;
  • Financially accessible health care resources;
  • Partnerships with primary and preventative care services.

MCC and ECMCC are seeking to engage multiple Community Based Organizations (CBO’s) to perform outreach within identified “hot spot” areas. CBOs located in “hot spot” areas will be trained by ECMCC on patient activation techniques, such as shared decision making and measurements of health literacy and cultural competency.
