Buffalo, N.Y. – Millennium Collaborative Care, PPS, in partnership with Olean General Hospital, will host a free Million Hearts® health screening event during the hospital’s Annual Strawberry Festival and Health Fair from 10 AM – 4:30 PM, Thurs., June 15, 2017 at OGH on 515 Main Street, Olean, NY.


As a part of the Million Hearts® screening event, attendees 18-years of age and older will have the opportunity to visit six stations including:

  • Blood Pressure Screening
  • Height and Weight
  • Patient Activation Measure® (PAM) Health Survey
  • Nutrition Education
  • Physical Education
  • Health Insurance Facilitation

“Cardiovascular disease, which includes heart attacks and strokes, is the leading reason for death across our eight Western New York counties,” said Kenneth Lee Gayles. M.D., Cardiologist, Gayles Medical; and Millennium’s Project Champion, Cardiovascular Disease/Million Hearts®.

“The goal of our Million Hearts® events is to help people understand and control their blood pressure,” added Priti Bangia, Population Health Manager, Millennium Collaborative Care. “This is so important because blood pressure if not treated, can result in people having heart attacks and strokes.”


After completing their testing and screening, a community navigator will also assist attendees with checking their eligibility for Medicaid, as well as help them find a primary care provider if they currently do not have one.


Heart disease and stroke are some of the top leading causes of death in the U.S. Statistics show that every 43 seconds someone has a heart attack in the U.S., as well as 1 American dies every 4 minutes from a stroke, according to millionhearts.hhs.gov.

Million Hearts® is a national initiative co-led by the Center of Disease Control (CDC) and Center for Medicare and Medicaid Services (CMS), with the goal of reducing the number of heart attacks and strokes by one million.

Millennium Collaborative Care (MCC), is a Performing Provider System (PPS) under New York State’s Delivery System Reform Incentive Payment (DSRIP) Program, a statewide initiative championed by Governor Andrew M. Cuomo to fundamentally restructure the health care delivery system by reinvesting in the Medicaid program, with the primary goal of reducing avoidable hospital use by 25% over 5 years.

The mission of Millennium Collaborative Care is to collaborate and partner with physicians, healthcare providers and community-based organizations on the implementation of 11 DSRIP programs designed by Millennium to positively impact Medicaid patient across Western New York’s eight counties (Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, Wyoming).
