The following team members have recently joined Millennium Collaborative Care during the previous quarter.

Christina Brooks, MHA, CCRP –  Practice Transformation Specialist

Prior to joining Millennium, Christina most recently served as System Support Specialist with Optimum Physician Alliance, Buffalo, NY. She holds an MHA, with a concentration in health policy and law from Capella University; a B.A. in Psychology from the State University of New York at Buffalo; and a BSc in Biomedical Sciences from the University of Guelph, Guelph, Ontario, Canada.

Peter Ciotta – Communications Director

Prior to joining Millennium, Peter most recently served as Regional Operations Communications Manager for DuPont, serving manufacturing sites in the Northeast. He holds a Master of Arts, Strategic Communications and Leadership from Seton Hall University, South Orange, NJ; and a B.A. in American Studies/Journalism from the University of Notre Dame, South Bend, IN.

Laura Kennedy — Practice Transformation Specialist

Prior to joining Millennium, Laura most recently served as Provider Liaison/Team Lead with Optimum Physician Alliance, Buffalo, NY. She holds a B.A. in Psychology from the State University of New York College at Buffalo (Cum Laude.)

Nina Obot — Clinical Integrations Coordinator

Nina most recently served as a Patient Navigator. She holds a Master of Science in Multidisciplinary Studies from the State University of New York College at Buffalo; and a B.S. in Biology from North Carolina Agricultural and Technical State University, Greensboro, NC.

Ebony Patterson-White — Community Health Worker

Ebony also most recently served as a Patient Navigator. She has over 15 years’ experience working directly in the community providing advocacy, education, and coordination of care in regards to behavioral, developmental disabled, and geriatric members of our community. She received her education in the City of Buffalo and studied at Erie Community College.

Amy Pease, RN. MA–  Practice Care Coordinator

Prior to joining Millennium, Pease most recently started her own healthcare consulting business and has contracts with WNY HEALTHeLINK and a national firm. She has more than 15 years working in and managing outpatient medical and dental clinics. She holds an MA, with a concentration in healthcare administration from Empire State College; an A.A.S. in Nursing from Jamestown Community College, Jamestown, NY.

 

 

Millennium also welcomed the following Interns:

Kailani Ameele is currently pursuing a B.S. in Health Care Studies at Daemen College, Amherst, NY.

Ju Joh is currently pursuing a Master of Public Health from the State University of New York at Buffalo School of Public Health and Health Professions. He holds a Doctor of Medicine from the University at Buffalo School of Medicine and Biomedical Sciences; and a B.S. in chemistry and biochemistry with honors, and a Master of Science in Chemistry from Stony Brook University, Stony Brook, NY.

Yitchuin Wong is currently pursuing a Master of Business Administration from the State University of New York at Buffalo and holds a Bachelor of Business Administration from Baruch College, Zicklin School of Business, New York, NY.
