Within Western New York, Cardiovascular Disease (CVD) is the number one cause of death across the eight counties and the mortality rate is higher than the national average. That is why high quality preventative care, clinical management of heart disease and strokes, and addressing the risk factors (including tobacco use), are top priorities at the state and regional levels.

The New York State Prevention Agenda Dashboard emphasizes the importance of preventing chronic disease in clinic and community based settings. DSRIP and NYSDOH will ensure collaboration and work together on Chronic Disease Prevention projects. This partnership along with regional entities will be critical to supporting the Million Hearts® goals.

Millennium Collaborative Care (Millennium) is working in partnership with agencies and health system partners across the eight Western New York counties to promote heart health through its Cardiovascular Health Project.

This Project is aligned with the national Million Hearts® initiative launched to bring communities, health care professionals, agencies and private sector organizations around a common goal – To prevent one million heart attacks and strokes by 2017.

For millions of Americans who are at high risk for heart attacks and strokes, (and may not know it), Million Hearts® has a number of initiatives that can be controlled through healthy lifestyle modifications.

As Millennium works with the local departments of health and its network partners in pursuing the goal of lowering cardiovascular disease, we will also work closely with the WNY population and the healthcare professionals who care for them. This means working closely with partner health systems to deploy electronic health record for cardiovascular disease based performance measurement and reporting, and adopting clinical decision support tools and patient registries in reaching out to patients for gaps in care.

To support community based programming, Millennium will also engage faith based programs, work with community health workshop providers and develop a process for referrals to community services, health homes, other providers to help improve heart health and blood pressure control.

As Millennium looks to transform the healthcare delivery system, we will work in partnership with our regional healthcare systems to support the Million Hearts® goals.  The use of best clinical practices, evidence based guidelines for Cardiovascular Disease and health information technology solutions will be used to drive, measure, and evaluate the effectiveness of our efforts to connect our Medicaid population to improving heart health.
