Reaffirming ‘People First’; More System Transformation, Clinical Integration

Man we’re living in interesting times!
Healthcare in America has been on a roller coaster ride; and the ups and downs of change keep coming at us at what seems like a mile a minute! No doubt, it’s been easy to get consumed with policy and process; and how new programs like “Pay-For-Performance” are going to work. These are all important. But, as I was reminded recently by a re-wind of New York State Medicaid Director Jason Helgerson’s remarks at our May Population Health Conference, “HOW” we are working to improve health care delivery is not MOST important. Population Health Conference Thank You !!!

Sincerest thanks to all Partners for attending and enthusiastically engaging in our recent Population Health Conference! Overall, the Conference exceeded our expectations, and the feedback we continue to receive has been overwhelmingly positive. There were two major anchor talks that drove the day. Dr.Jon Burroughs’ presentation on Population Health and what it means on a global scale; and New York State Medicaid Director Jason Helgerson’s message around DSRIP and how that connects to Population Health. Both received great reviews. The third was the Value Based Panel that was facilitated by Ann Monroe, Co-Chair NYS DSRIP Project Approval and Oversight Panel and Former President. Health Foundation for Western and Central New York, that also received excellent feedback. |

Partners Answering Need, Expanding Behavioral Health Treatment Access

Behavioral Health Integration continues to be one of single largest initiatives at Millennium Collaborative Care.
In our Q4 Update, we discussed our collaborative work with local Behavioral Health providers and primary care physicians to develop community best practices for depression and substance abuse. In response to the growing needs of the primary care community for better access to behavioral health treatment, Millennium’s Behavioral Health Delivery System Reform Incentive Payment Program (DSRIP) Partners have developed and expanded on open access intake hours to expedite the process to get a patient into treatment. Building Up ‘Patient Centered Medical Neighborhoods’

In our DY2, Q4 Update, I began to introduce the concept of the “Patient Centered Medical Neighborhood” (PCMN) as an emerging priority for the Millennium Clinical Integration Team.
The Agency for Healthcare Research and Quality (AHRQ) articulates that a successful “Medical Neighborhood” will “focus on meeting the needs of the individual patient, but also incorporate aspects of population health and overall community health needs.” The “Medical Neighborhood” is defined as a clinical-community partnership that includes the medical and social supports necessary to enhance health, with the PCMH practice serving as the patient’s primary “hub” and coordinator of health care delivery.

Regional Clinical Integration Incentive Program Planned for Fall

In early Fall of this year, Millennium will be launching a new regional Clinical Integration Program highlighted by an opportunity to earn substantial incentive funds.
The program will be established in each of three regions: Southern Tier; Niagara County; Erie County. It will be open to Partners within each respective region including: Regional Hospital; Primary Care Providers and Clinics that serve a large number of Medicaid recipients; and Behavioral Health Agencies/Providers within each respective region. Turning Data into Timely, Actionable Information

Over the past year, we have been working with our service providers to unlock the patient data they capture in their electronic medical record (EMR) systems and share it with Millennium to help improve population health.
This is being done in one of two ways: either by sending clinical document summaries to HEALTHeLINK, or by sending their clinical records directly to Cerner HealtheIntent, our population health management platform. HealtheIntent aggregates the data and provides the tools to help our network manage the health of our Medicaid members. This is done through chronic disease and wellness registries; a care management tool and an enterprise data warehouse that we can use to build reports and perform data analysis. Ultimately, it is Millennium’s responsibility to turn this vast web of disparate clinical data into actionable insights that our providers can use to improve the health of our community residents. Addressing Barriers; Improving Provider – Client Communications
Key Steps Towards System Transformation, Clinical Integration

At International Nursing Conference in Dublin Ireland
Million Hearts®, a community-based cardiovascular health initiative (project 3.b.i.) led by Millennium Collaborative Care, PPS in partnership with Greater Buffalo United Ministries (GRUM) and the University at Buffalo School of Nursing, was awarded a 2016 Spotlight on Population Health Initiatives (SOPHi) Award by P² Collaborative of Western New York at the 2nd annual SOPHi Awards held last October.
