Guided by Michele Mercer, RN, MSHA, Chief Clinical Integration Officer, Millennium Collaborative Care’s Population Health Services Team provides care coordination
support and enhanced patient services to our Primary Care partners including:

• Address gaps in care

• Assist with meeting quality and performance-based metrics

• Helping physicians to work with their patients in attaining disease state goals
(e.g. A1c, BP)

• Improve medication adherence and patient education

• Improve medication reconciliation process within primary care practices

• Decrease discrepancies found in patient medication lists

• Identify and address gaps in care for Social Determinates of Health-related
concerns

• Strengthen partnerships with Community Based Service providers

• Help patients connect to needed community resources

• Provide partners with the tools and support for their patients to become
advocates for their own health

Millennium Collaborative Care, a Performing Provider System (PPS), is igniting healthcare change by partnering with physicians, healthcare providers and community-based organizations to improve the delivery of healthcare for Medicaid patients across the eight counties of Western New York. The Erie County Medical Center (ECMC) is the lead entity in the Millennium PPS.
