Millennium’s annual Community Engagement Plan strives to implement five community health opportunities per year. Thus far, in DSRIP Year 2 (which concludes March 31, 2017), that goal has been exceeded as 12 programs have been implemented!

Highlights of four recent events include:

Enterprise Charter School ‘Night of Wellness’

Enterprise Charter Students and their families participated in a “Night of Wellness” Nov. 14 at the school located at 275 Oak Street in downtown Buffalo which featured a wide range of workshops designed for both children and adults including sessions focused on:

  • Mental Health
  • Health
  • Fitness
  • Nutrition
  • Dental Health

A wonderful outcome of the event has been the development of a new partnership between Enterprise Charter and the UB School of Dental Medicine that will provide free dental appointments this Spring and Fall for the school community.

Community Conversations

In partnership with Niagara Falls Memorial Medical Center, a very successful “Community Conversation” was held Nov. 10 at Abate Elementary School in Niagara Falls

The Conversation focused on “Mental Health Disorders and Substance Abuse in Young Women” including best practices expectant mothers should follow during pregnancy to ensure your child has a good start.

In addition to NFMMC, the collaborative effort was supported by the Community Health Center of Niagara, Community Missions, Mental Health Association in Niagara County, Millennium Collaborative Care, Niagara County Department of Mental Health, Niagara Falls City School District, Niagara Falls Memorial Medical Center, Northpointe Council, P2 Collaborative and Niagara County Legislator Owen Steed. This team continues to meet to determine next steps to support the community in this key area.

‘Community Celebrations’

In partnership with Jericho Road Community Health Center ( ), Millennium assisted with the launch of a new program called “Community Celebrations”.

The “Celebrations” are educational programs offered multiple times per year featuring rotating topics that are designed to empower families. The health-related topics include:

  • Immunizations
  • Urban Gardening
  • Home Safety
  • Emergency Planning
  • Oral Care for Children
  • Developmental Milestones

Save the date: February 15, 2017 ‘Health and Wealth’ Event

A second “Health and Wealth” event presented in partnership with Buffalo Municipal Housing Authority and multiple community partners will be held from 10 AM to Noon, Wed., Feb. 15, 2017 at the Belle Center ( ), 104 Maryland St, Buffalo, NY 14201.

The event will focus on job readiness, job search best practices, and offer overall assistance with the objective of securing employment; a social determinant to living a healthy life.

The intent of all Millennium Community Engagement efforts is collaboration with regional organizations to raise the profile of Millennium’s goals and meet regional health and wellness needs. If you are interested in partnering to address the needs in your community, please email

The mission of the show is to support Millennium’s Community Engagement efforts including:

  • Provide community awareness about Millennium Collaborative Care projects, initiatives and goals.
  • Solicit community involvement in reducing avoidable hospitalization and emergency room use through awareness, education and empowerment.
  • Provide health education and information designed to engage and empower community members in improving their health.
  • Inform community on population health statistical data to enlighten and inform on local prevalence of disease and prevention as a strategy to motivate behavior modification.
  • Introduce community to expert information regarding mental health and health issues as well as resources for help and treatment.
  • Inform and educate about health root causes and social determinants of health.
