‘Re-Engineered Hospital Discharge Program Aimed at Lowering High Rates of Potentially Avoidable Readmissions

According to the Western New York Community Needs Assessment which was completed in 2014, our baseline Potentially Avoidable Readmission rate is 5.8/100 patients. Niagara, Orleans, and Chautauqua Counties are nearly double the goal rate, with a rate as high as 8.9/100 patients. Read More

Christine Blidy Appointed Chief Network Officer, Millennium Collaborative Care

Christine Blidy has been appointed Chief Network Officer effective Dec. 3. As Chief Network Officer, Blidy will serve as the primary liaison to Millennium’s partners including physicians, hospitals and other healthcare providers. Read More

 

Millennium Organizational Changes

Michele Mercer, Chief Clinical Integration Officer, announced the following organizational changes effective Dec. 5, 2016:

Janet Stoeckl, Clinical Director has a new team which includes:  Priti Bangia, Project Performance Manager; Annie Deaver, Acute Care Manager; and Saralin Tiedeman, Post-Acute Care Manager.  Nina Obot, Clinical Integration Coordinator, will continue to report to Annie Deaver. Janet will continue to report directly to Michele Mercer.

Andrea Wanat, Director Behavioral Health has a new team which includes: Kimberly Backey, Cultural Competency and Diversity Manager, and LaTonya Diggs, Behavioral Health Manager. Andrea will continue to report directly to Michele Mercer.

Rita Hubbard–Robinson, (Consultant) Project Director Patient Activation Measure (PAM), has begun working with the Community Engagement Team which includes Sandra McDougal, Project Manager, Maternal Child Health; Rachel Laster, Community Outreach Coordinator; and Ebony Patterson-White, Community Health Worker Coordinator.  Both Rachel and Ebony report directly to Sandy.  Rita will report directly to Michele Mercer.

Dr. Anthony Billittier, Chief Medical Officer announced the following organizational changes: Jillian Barone-Greenbaum has been promoted to Director of Ambulatory Care Transformation and Provider Engagement.  Jillian’s team includes Laura Kennedy, Practice Transformation Specialist; Christina Brooks, Practice Transformation Specialist; and Amy Pease (contract) Practice Transformation Specialist. Jillian will continue to report directly to Michele Mercer.

Dr. Kenyani (Kiki) Davis, Internal Medicine MD – UB/MD, has been contracted as part-time Assistant Medical Director, Millennium Collaborative Care and Millennium Collaborative Care Champion: PAM project. She will report directly to Michele Mercer.
