Ebony M. White has been appointed Community Health Worker, Population Health Services, effective Mon., Nov. 26, 2018.

Millennium is very pleased to welcome Ebony back to the organization and the Community Health Worker role where did an outstanding job supporting and leading numerous initiatives including Emergency Department Patient Navigator training; serving as a Patient Activation Measure/Trauma Informed Care trainer; community engagement relationship coordination; and managing Millennium’s Barber and Beauty Shop Blood Pressure Self-Monitoring initiative.

Ebony has also served as a Community Health Worker for University Emergency Medical Services; and as a Direct Support Professional for Community Services for the Developmental Disabled.

In addition to holding an Associate degree in Community and Human Services from Empire State College; Ebony has earned distinction as a Trauma Informed Champion, Trauma Informed Care Institute of Buffalo at the University at Buffalo School of Social Work; Facilitator for the United Way of Buffalo Economic Poverty Initiative Community Conversation; and Peer Leader, Erie County Department of Senior Services Living Healthy Self-Management.
