Open Forum to also discuss ‘Behavioral/Mental Health’; Provide Resources on site

Millennium Collaborative Care has announced “Buffalo Health Matters!…A Community Conversation” will take place Thurs., May 26, 2016 at Bennett High School, 2885 Main St., Buffalo, NY.

The “Conversation” is free and open to the public. Doors open at 5 PM with speaking program to begin at 5:45 PM. Free parking is available adjacent to Bennett High School and light refreshments courtesy of Tops Markets “Neighbor Helping Neighbor” program will be served.

The “Conversation” is a collaborative effort between Millennium and:

Organized in response to the ongoing issue of opiates/prescription abuse – with focus on the effect on communities in the City of Buffalo – the forum will also include discussion on “Mental/Behavioral Health” and “Suicide Prevention”.

Provider resources will also be present to meet with attendees following a program of speakers including:

A Catalyst for Healthcare Change in Western New York

Millennium Collaborative Care (MCC) is a Performing Provider System (PPS) under New York State’s Delivery System Reform Incentive Payment (DSRIP) program, a statewide initiative championed by Governor Andrew M. Cuomo to fundamentally restructure the health care delivery system by reinvesting in the Medicaid program, with the primary goal of reducing avoidable hospital use by 25% over 5 years.

The mission of Millennium Collaborative Care is to collaborate and partner with physicians, healthcare providers and community-based organizations on the implementation of 11 DSRIP programs designed by Millennium to positively impact Medicaid patients across Western New York’s eight counties (Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, Wyoming).
