Millennium Collaborative Care rolled out the first wave of Master Participation Agreements to attested Safety-Net Providers including primary care, skilled nursing facilities, behavioral health/substance abuse and developmentally disabled focused providers and home health agencies.  This first group of providers will receive a combined $10.6 million based on their Year 1 participation in the 11 DSRIP Projects. Payment is based on a matrix broken into three types of activities, engagement at 70%, reporting at 20% and performance at 10%.
