Spotlight CBOs: Buffalo Federation of Neighborhood Centers; Cattaraugus Community Action; Erie County Council for the Prevention of Alcohol and Substance Abuse; Housing Options Made Easy; Jewish Family Service; Salvation Army; Western New York Independent Living
With the primary objective of building community health infrastructure in preparation for value-driven reimbursement; Millennium Collaborative Care throughout 2019 and into 2020 has partnered with over 50 Community Based Organizations (CBOs) who participated in three Millennium-developed programs — Value-Based Proposition; Value-Based Enrichment; GetSET Readiness.
To date, two programs – Value-Based Proposition and Value-Based Enrichment have been successfully completed. Meanwhile, Millennium’s Value-Based GetSET Readiness Program – the most robust of the three – continues with the enrolled CBOs more than halfway through an intensive 24-month intensive syllabus based on the Health Foundation for Western & Central New York model. (The participating GetSET CBOs represent the eight counties of Western New York. CBOs were evaluated on their ongoing ability to support integrated, person-centered solutions and health outcome improvement in addition to the direct services the CBOS provide relating to social determinants of health (SDOH): Food, Housing, Transportation, Childcare, Employment, and Education.
Begun in January of 2019, the GetSET program includes:
• Assignment of a Consultant to help the organization with capacity-building plan and value proposition statement.
• Assessment of organizational strengths/weaknesses in eight core competency areas.
• Development and implementation of a capacity-building plan.
• Development/enhancement of the organization’s value proposition(s).
• Group-based education sessions on relevant topics and issues.
• Peer mentoring.

“The primary objective of the Value-Based GetSET Readiness Program is to support integrated, person-centered solutions and health outcome improvement,” explained Kimberly Backey, MBA, Director of Population Health Services and SDOH. “Ultimately, Millennium is striving to prepare our agency partners to participate in the future state—an integrated delivery system that is being transformed through Medicaid redesign and is expected to bear risk through payment reform.”
