Checking blood pressure

Millennium Collaborative Care, PPS, in partnership with Greater Buffalo United Ministries and the University at Buffalo School of Nursing, will be hosting three Million Hearts® health screening events throughout the month of July at Buffalo-area churches and the University at Buffalo South Campus.

Adults 18-years of age and older can visit the following for free health screenings:

Sat., July 15, 2017 10 AM – 2 PM JFK Park at Friendship Baptist Church

Clinton and Hickory Street, Buffalo, NY

Wed., July 19, 2017 6 PM – 8 PM ‘UB on the Green Care Night’

UB South Campus in front of Hayes Hall

3435 Main Street, Buffalo, NY

Sat., July 22, 2017 11 AM – 5 PM Miracle Missions Full Gospel Church

In addition to blood pressure screening by UB Nursing Students, attendees will visit three additional stations including: Personal Health Survey; Height and Weight; along with education and resources for nutrition and exercise.

“Cardiovascular disease which includes heart attacks and strokes is the leading reason for death across our eight Western New York counties,” said Kenneth Lee Gayles. M.D., Cardiologist, Gayles Medical; and Project Champion, Cardiovascular Disease/Million Hearts®.

“The goal of our Million Hearts® events is really is to help people understand and control their blood pressure,” added Priti Bangia, Population Health Manager, Millennium Collaborative Care. “This is so important because blood pressure if not treated, can result in people having heart attacks and strokes.”


After completing their testing and screening, a community navigator will also assist attendees with checking their eligibility for Medicaid, as well as help them find a primary care provider if they currently do not have one.


Heart disease and stroke are some of the top leading causes of death in the U.S. Statistics show that every 43 seconds someone has a heart attack in the U.S., as well as 1 American dies every 4 minutes from a stroke, according to

Million Hearts® is a national initiative co-led by the Center of Disease Control (CDC) and Center for Medicare and Medicaid Services (CMS), with the goal of reducing the number of heart attacks and strokes by one million.

Millennium Collaborative Care (MCC), is a Performing Provider System (PPS) under New York State’s Delivery System Reform Incentive Payment (DSRIP) Program, a statewide initiative championed by Governor Andrew M. Cuomo to fundamentally restructure the health care delivery system by reinvesting in the Medicaid program, with the primary goal of reducing avoidable hospital use by 25% over 5 years.

The mission of Millennium Collaborative Care is to collaborate and partner with physicians, healthcare providers and community-based organizations on the implementation of 11 DSRIP programs designed by Millennium to positively impact Medicaid patient across Western New York’s eight counties (Allegany, Cattaraugus, Chautauqua, Erie, Genesee, Niagara, Orleans, Wyoming).
