Program will air every Monday in October at 5:30 PM on Spectrum/Time Warner Cable

Millennium’s Saralin Tiedeman, MS, OTR/L, Population Health Clinical Performance Manager, was a guest on “Aging is for Everyone”; a WKBW Ch. 7 public service program hosted by Leilani Joven Pelletier, MS, Program Director, Center of Excellence for Alzheimer’s Disease, UB Jacobs School of Medicine and Biomedical Sciences.

The theme for the September show was “Transitions in Care”, featuring segments focused on hospital discharge planning, community-based case management, and a description of Millennium’s efforts at a system level to improve health care in Western New York, including Performing Provider System initiatives and linkages with all levels of service and support including federal health programs.

The program aired on Channel 7 at 6:30 AM on Saturday, Sept., 22, and will be broadcast every Monday in October at 5:30 PM on Spectrum/Time Warner Cable.
