Three Training Options Available

In efforts to prepare our Community Based Organization (CBO) partners to position themselves for success in 2019 relating to the New York State Department of Health Value Based Payment (VBP) Roadmap, Millennium Collaborative Care has developed three unique programs driving VBP technical assistance.

Option 1 – The Value Based Readiness Program

Starting January 2019, this 15 month program focuses on helping organizations begin to strengthen their internal infrastructure at the operations level, and address sustainability in the value-based payment world.

This program is designed to work exclusively with an Organizational Development Consultant to assist with; assessing the organizational strengths/weaknesses in eight core competency areas, development and initial implementation of a capacity-building plan, and development/enhancement of the organization’s value proposition statement.

Option 2 – Value Based Proposition Program

This is a collaborative effort with Community Partners of WNY that includes a two (2) group training program, designed to assist organizations to identify and develop their value proposition(s) and recognize and respond to business opportunities.

Selected organizations will receive technical assistance from Value Base Proposition expert Kristen Woodlock, CEO of Woodlock & Associates.

• Option 3 – Value Based Enrichment Program

Starting February 2019 – May 2019, this four (4) month program features facilitated sessions led by the MCC’s Team and guest speakers in preparing  participating CBO’s for VBP. The program is designed to assist organizations  to identify and develop their value proposition(s) and recognize and respond to business opportunities. 

Program includes; Value Based Payment Webinars, technical assistance on Value Based Propositions from MCC Staff and guest speakers, peer mentoring, and group training on creating value propositions.

Each program is designed to strengthen the CBO’s ability to participate in an outcome driven environment through Value Base Payment Education & Training.

Interested CBO’s must provide direct services related to the Social Determinants of Health (SDOH) with a focus on food, housing, transportation, childcare, employment and/or education.

Organizations must also indicate on the Community Health Capacity and Value Based Readiness Programs Application (PLEASE CLICK HERE FOR APPLICATION) their desired program option in order of need.
