This special issue of Buffalo Healthy Living Magazine is an important example of what Millennium Collaborative Care is trying to accomplish in the priority area: Social Determinants of Health.
We are working with our health partners to emphasize the importance of eating fresh, whole foods. To accomplish this, we are collaborating with local growers, dieticians,
and food markets to ensure that more people understand the ways that food, both healthy and unhealthy, can impact an individual’s health. We want to emphasize that
healthy eating can make a significant difference in how you feel and how it can impact you and your family in helping to keep you disease-free.
