Enhancing behavioral healthcare outcomes by working with partner agencies across eight counties with a focus on sharing critical patient information
Behavioral Health Crisis Response
Millennium’s Behavioral Health Team is striving to collaborate with our partners to provide readily accessible behavioral health crisis services across WNY’s eight counties that will allow patients access to an appropriate level of service and providers.
We Are Creating Change…
To address growing crises, Department of Mental Health Commissioners in all eight counties have been essential in building a comprehensive gaps and barriers analysis for hotline, mobile outreach, respite/peer services and substance abuse services.
In addition, representatives from partner agencies such as Crisis Services, HOME, Buffalo Psychiatric Center, Community Missions, Restoration Society, and Northpointe Council have been participating in bi-monthly workgroups to pinpoint needs and gaps and to prioritize solutions to fill in the gaps. Millennium representatives are also participating in Opiate Task Forces in both Erie and Niagara counties; and are working with our Finger Lakes Region PPS partner to develop a coordinated plan to fill service gaps.
Primary Care and Behavioral Health Integration
Millennium’s Behavioral Health Team is working with our partners to integrate mental health and substance abuse with primary care services to ensure coordination of care within the population requiring these services. This integration effort includes assessments by Millennium Transformation Specialists and the implementation of integration training plans.
Promote Mental, Emotional, and Behavioral Well-being
The overall objective is to expand evidence-based programs and practices in the schools and communities to help prevent substance abuse and promote positive mental health. In addition, a comprehensive public awareness campaign is in development that aims to deter negativity towards those with mental health and/or substance abuse issues.
We Are Creating Change…
Led by the Erie County Council for the Prevention of Alcohol and Substance Abuse and the Mental Health Association of Erie County, the Millennium Behavioral Health Team is working alongside Community Partners of WNY to ensure the expanded delivery of a comprehensive array of school and community-based educational programs across our eight county region. Partner agencies include Northpointe Council, The Chautauqua Council on Alcoholism and Substance Abuse, CAReS – Council on Addiction Recovery Services, Genesee-Orleans Council on Alcoholism and Substance Abuse (GCASA), WNY United Against Drug and Alcohol Abuse, and PreventionFocus.
