Janet Stoeckl, RN, CCM
As Clinical Director for Millennium Collaborative Care, Janet Stoeckl leads the Ambulatory Services team, including MCC’s overall Patient Centered Medical Home (PCMH) transformation efforts.
She has extensive experience providing PCMH leadership which seeks to develop close alignment between primary and specialty care physicians with the goal of improving the patient care experience and addressing inefficiencies that result in redundancy and contribute to rising healthcare costs.
Prior to joining Millennium, Stoeckl served for 19 years at Amherst, NY-based insurer Independent Health where she developed and expanded “The Primary Connection,” an initiative to enhance collaboration between Primary Care and Specialty Care physicians. She also led IH’s “Medical Innovation and Specialty Advisory” teams that collaborated with physicians to identify drivers of cost in primary and specialty care, and implemented key initiatives to decrease variation in clinical practices. Stoeckl also deployed IH’s high risk prenatal case management program, and general case and disease management programs. She received Nurse of Distinction at Independent Health.
Stoeckl also has extensive experience in cost containment, budget forecasting, regulatory compliance, National Quality Association (NCQA) and project management. She is a Registered Nurse and Certified Case Manager. A graduate from Millard Fillmore School of Nursing, Stoeckl also attended Empire State College.
