Million Hearts® Health Screening Event

October 21 @ 10:00 am - 1:00 pm

Millennium Collaborative Care, PPS, in partnership with Greater Buffalo United Ministries and the University at Buffalo School of Nursing, will be hosting Million Hearts® health screening events throughout the month of October at Buffalo-area churches.

SIX HEALTH STATIONS

In addition to blood pressure screening by UB Nursing Students, attendees will visit three additional stations including: Personal Health Survey; Height and Weight; along with education and resources for nutrition and exercise.

HEALTH INSURANCE/ PRIMARY CARE FOLLOW UP

After completing their testing and screening, a community navigator will also assist attendees with checking their eligibility for Medicaid, as well as help them find a primary care provider if they currently do not have one.

HEART DISEASE AND STROKE NATIONALLY

Heart disease and stroke are some of the top leading causes of death in the U.S. Statistics show that every 43 seconds someone has a heart attack in the U.S., as well as 1 American dies every 4 minutes from a stroke, according to millionhearts.hhs.gov.

Million Hearts® is a national initiative co-led by the Center of Disease Control (CDC) and Center for Medicare and Medicaid Services (CMS), with the goal of reducing the number of heart attacks and strokes by one million.
